Confidential Mental Health Screening
Welcome to Wellesley Health Department's online screening survey for mental health disorders. This web-based tool allows you to take an anonymous self-assessment for depression, bipolar disorder, generalized anxiety disorder, and posttraumatic stress disorder. You will receive immediate feedback and resources after completing a screening.

Screening provides a quick way to identify whether you or a family member may be experiencing symptoms associated with an illness and whether follow-up with a professional may be beneficial. We encourage you to take advantage of this service without obligation and you can be assured all responses are strictly confidential.


Learn to recognize the warning signs of suicide – Question, Persuade, Refer training available
According to the QPR Institute, QPR stands for Question, Persuade, and Refer - three simple steps that anyone can learn to help save a life from suicide. Just as people trained in CPR and the Heimlich maneuver help save thousands of lives each year, people trained in QPR learn how to recognize the warning signs of a suicide crisis and how to question, persuade, and refer someone to help.
